  • Kindly prepare the following documents to avoid inconveniences:
    1. A copy of the deposit slip/transaction details for the registration fee of PhP 500.00.
      Payment can be done through Mobile Banking transfer, Over-the-counter Deposit, or via cash accept machines through our BPI Account:
    2. A scanned copy of your 2×2 ID Picture
    3. A scanned copy of your PRC ID

  1. A copy of the deposit slip/transaction details for the renewal fee of PhP 500.00.
    Payment can be done through Mobile Banking transfer, Over-the-counter Deposit, or via cash accept machines through our BPI Account.
  2. Save the your file in the following format: Last Name, First Name, Middle Initial
  3. Fill up the linked concise google form for our record.
